Discectomy

A lumbar discectomy is a surgical procedure performed to remove a damaged or herniated disc in the lower back. Intervertebral discs act as cushions between the vertebrae, and when a portion of a disc presses on a nerve root or the spinal canal, it can cause pain, numbness, or weakness. During the surgery, the affected portion of the disc is removed to relieve pressure on the nerves and reduce the risk of future disc herniation.

The procedure is performed under general anaesthesia to ensure comfort and pain control. A small incision is made in the lower back, and in some cases, a microdiscectomy is used—a minimally invasive approach with a surgical microscope to precisely remove the damaged disc while minimizing tissue disruption.

Physiotherapy Before Lumbar Discectomy

Pre-operative physiotherapy helps prepare your back for surgery by teaching you exercises to stretch and strengthen the muscles that support your spine. These sessions aim to optimise your physical condition, making post-surgery recovery smoother and more effective. Symptoms Following a Lumbar Discectomy

After a lumbar discectomy, it is normal to experience some pain and discomfort in the lower back. Most patients are able to get out of bed within a few hours after surgery, with guidance from a physiotherapist on safe and comfortable movement. Your physiotherapist will teach you proper techniques for transferring and walking, ensuring you can mobilise safely at home.

Discharge usually occurs once your condition is stable and your physiotherapist is confident in your ability to move independently. You will receive advice on activities to avoid during the first few weeks, such as bending, lifting, or twisting. Gentle exercises will be provided to continue at home, helping to support healing, reduce stiffness, and promote a smooth recovery.

Physiotherapy Following a Lumbar Discectomy

In the first one to two weeks post-surgery, physiotherapy focuses on managing pain and inflammation while promoting soft tissue healing. Treatment at this stage may include:

2–6 Weeks After Surgery

During this stage of recovery at Ace Physio Sports, you will begin a structured exercise program guided by your physiotherapist. Activities such as walking, swimming, and stationary cycling are recommended to improve fitness, increase energy levels, and support overall recovery.

Your treatment will also focus on strengthening and controlling the muscles that stabilise your lower back, helping you gain confidence and independence with daily activities. Guidance will be provided on safely lifting, carrying, and performing more strenuous tasks. You will also learn safe techniques for getting in and out of a car, preparing for a gradual return to driving. Light work can typically resume within 2–4 weeks, depending on the nature of your occupation.

Rehabilitation during this phase may include:

  • Walking, swimming, and stationary cycling
  • Stretching exercises to increase flexibility
  • Exercises to improve muscle control and core stability
  • Guidance on posture and safe movement during daily activities such as lifting, bending, and reaching

6–12 Weeks After Surgery

At this stage, your physiotherapy program will progress to further strengthen the muscles around your lower back and increase range of movement. Your physiotherapist will also provide advice on workplace posture and positioning to enhance comfort during tasks.

Treatment may include:

  • Abdominal and core strengthening exercises
  • Gradually increasing walking distances
  • Work- or sport-specific exercises
  • Continuing swimming, walking, and cycling to maintain fitness and manage weight

Patients with physically demanding jobs may be advised to consider temporary modifications or alternative duties.

12+ Weeks After Surgery

In this final stage, your rehabilitation will focus on improving confidence with daily tasks at home and at work, ensuring all activities are performed safely and effectively. Continued adherence to your prescribed exercise program is essential to optimise recovery, regain full function, and safely return to work or recreational activities.
