Decompression

Lumbar decompression

Lumbar decompression surgery involves removing bone or disc material from the lower spine to relieve pressure on the nerve roots. This procedure helps create more space for the spinal nerves, reducing pain and discomfort in the lower back, buttocks, and legs while restoring normal movement and function. The most common types of lumbar decompression surgeries are microdiscectomy and laminectomy, both aimed at easing nerve compression. It is typically recommended for individuals who experience persistent pain, numbness, weakness, or difficulty walking. Performed under general anaesthesia, the surgery usually requires only a short hospital stay.

Physiotherapy Before Lumbar Decompression

Pre-operative physiotherapy plays a key role in preparing your body for lumbar decompression surgery. It focuses on strengthening and stabilising the spine while improving flexibility and overall conditioning. Gentle exercises are customised based on your current pain levels and mobility to ensure your back muscles are well-prepared for recovery after surgery. Your physiotherapist will also provide valuable guidance on what to expect post-surgery and how to manage your back safely during the initial recovery phase.

Symptoms After Lumbar Decompression

Following lumbar decompression surgery, it is common to experience mild to moderate pain and discomfort as part of the normal healing process. Tenderness around the surgical site is expected and can be managed with prescribed pain medication or the application of ice packs for relief. In most cases, a physiotherapist will assist you in standing and walking within 24 hours of surgery, depending on your comfort level. Early movement is encouraged to aid healing and prevent stiffness. Most patients are discharged within 3 to 6 days, once the physiotherapist is satisfied with your ability to walk and move independently.

Physiotherapy following a lumbar decompression

Physiotherapy should begin as soon as possible after your lumbar decompression surgery to ensure optimal recovery and long-term results. At Ace Physio Sports, our physiotherapists focus on identifying and correcting any post-surgical movement issues, muscle imbalances, or joint stiffness that may have developed. Your rehabilitation program will include targeted exercises to improve mobility, strengthen core and back muscles, enhance flexibility, and restore overall physical function. With guided physiotherapy support, you can safely return to your daily activities and regain confidence in your movement.

1–2 Weeks After Surgery

Physiotherapy at Ace Physio Sports begins soon after your surgery to support recovery and help you gradually return to daily activities or work. During this early stage, treatment focuses on gentle, guided movements and pain relief to promote healing and confidence. Your physiotherapy sessions will include:

  • Training on how to move and walk safely and independently
  • Guidance on maintaining correct posture and improving movement confidence
  • Pain and inflammation management through electrotherapy and hands-on techniques
  • Gentle range of movement exercises to prevent scar tissue formation and support healing
  • Tailored pain management strategies

Your physiotherapist will also provide advice on which activities to avoid while your pain subsides and your spine continues to heal, ensuring a safe and effective recovery process.

2–6 Weeks After Surgery

During this phase of recovery, your physiotherapy at Ace Physio Sports will focus on gradually increasing activity levels and building strength through a structured and personalised exercise program. You will also be encouraged to continue certain exercises at home to maintain progress between sessions. Walking remains one of the best activities at this stage and will be included in your daily routine.

Physiotherapy treatment during this period may include:

  • Re-educating proper and pain-free movement patterns
  • Stretching and mobilising muscles and joints to restore full range of motion
  • Isometric core and abdominal strengthening exercises
  • Strength training for the quadriceps, hamstrings, and gluteal muscles
  • Walking and stationary cycling for endurance
  • Hydrotherapy sessions to enhance flexibility and muscle control
  • Guidance on self-managing daily activities safely

Most patients can expect to return to work and driving between 4–6 weeks, depending on individual progress. Your physiotherapist will closely monitor your movement, balance, and strength to ensure a safe and confident return to normal routines.

6–12 Weeks After Surgery

At this stage, your rehabilitation program will advance to further strengthen the muscles supporting your lower back, hips, and legs. Your physiotherapist will customise exercises to match your work and lifestyle needs, ensuring a smooth return to normal and sporting activities.

Physiotherapy during this period may include:

  • Strengthening deep abdominal and gluteal muscles to stabilise the spine
  • Stretching to maintain flexibility and reduce stiffness
  • Increasing walking distance and endurance
  • Incorporating work- or sport-specific exercises
  • Correcting posture in sitting, standing, and lying positions for long-term comfort

12+ Weeks After Surgery

At this stage of your recovery, your physiotherapist at Ace Physio Sports will focus on further enhancing your muscle strength, flexibility, endurance, posture, and overall functional ability. Your treatment program will be tailored to your individual needs to help you achieve your maximum physical potential and safely resume the activities that matter most to you.
