Mobilisation Under Anesthetic - MUA

A manipulation of the ankle under anaesthetic (MUA) is a procedure sometimes performed following an ankle fracture by an orthopaedic consultant. This procedure is indicated if the bones of the ankle have moved out of alignment after the fracture. The purpose of an MUA is to realign the displaced ankle bones before a cast is applied, ensuring the bones heal in a satisfactory position. The procedure is typically performed under local anaesthetic, requires no incisions, and therefore carries minimal risk of infection. An MUA is usually considered when the displacement is minimal, or if the individual is at higher risk of complications from general anaesthesia or surgery, such as in elderly patients or those with heart or lung conditions. If the outcome of the MUA is unsatisfactory and the patient is fit for surgery, an open reduction internal fixation (ORIF) may be recommended to properly realign and stabilise the ankle joint.

Following approximately 8 weeks in a cast (this may vary depending on the individual), you will be ready to begin physiotherapy. In our experience, once you have received the all-clear from your orthopaedic consultant, the sooner physiotherapy is started, the better the recovery outcome following a broken ankle.

Symptoms after Ankle Manipulation Under Anaesthetic

If you have experienced a displaced ankle fracture followed by a manipulation under anaesthetic (MUA), you may be experiencing a range of symptoms, including:

  • Pain around the affected ankle
  • Swelling and bruising of the joint and surrounding tissues
  • Stiffness or reduced range of motion
  • Tenderness at the fracture site
  • Weakness in the ankle and lower leg muscles
  • Difficulty bearing weight or walking normally

Physiotherapy after ankle ORIF

After 6–8 weeks, you will have a follow-up appointment with your orthopaedic consultant, who will X-ray your ankle to assess whether the bones are healing adequately. Based on this assessment, your consultant will determine your weight-bearing status. At this stage, your physiotherapy programme will focus on restoring movement in your ankle joint, improving range of motion, and gradually preparing you for functional activities.

Weeks 1-6

While you are wearing your cast, physiotherapy will focus on maintaining strength and flexibility in other joints and muscle groups. No exercises or movements will be performed on your affected ankle during this time to allow for proper healing. Other physiotherapy interventions during this period may include:

  • Strengthening exercises for the knee, hip, and core muscles
  • Range of motion exercises for unaffected joints
  • Circulation and oedema management for the leg and foot
  • Education on safe mobility and techniques for moving around with the cast

Weeks 6-12

As soon as your cast is removed, active rehabilitation can begin with Ace Physio Sports. At this stage, rehabilitation focuses on mobilising your ankle, restoring weight-bearing ability, and strengthening the surrounding muscles. Physiotherapy is essential following an ankle fracture to ensure proper recovery and prevent long-term complications. Your physiotherapy programme at this stage may include:

  • Weight-bearing exercises to gradually increase strength and stability
  • Strengthening exercises for the calf, foot, and lower leg muscles

12 weeks and onwards

Your progress at this stage depends on factors such as the severity of the fracture, the duration of cast immobilisation, and your commitment to your home exercise programme. Physiotherapy at this stage may include:

  • Gait retraining to ensure a normal walking pattern
