What is a pulled hamstring?

A pulled hamstring (also known as a hamstring tear or hamstring strain) occurs when one or more of the hamstring muscles, located at the back of the thigh, are torn. Physiotherapy plays a crucial role in both the treatment and recovery of a pulled hamstring.

How does a pulled hamstring happen?

A pulled hamstring most commonly occurs when the muscle is forcibly contracted while in a stretched position, often during activities such as running or sprinting.

What are the symptoms of a pulled hamstring?

The first sensation when a hamstring muscle is torn is often sudden, sharp pain in the back of the thigh. This may be accompanied by a distinct ‘tearing’ sensation. In minor tears, you may be able to continue participating with only slight discomfort. However, as the muscle cools down, the pain often increases due to bleeding and swelling within the injured muscle. Other symptoms may include:

  • Tenderness when pressing on the back of the thigh
  • Bruising and discoloration in the affected area
  • Stiffness or difficulty moving the leg
  • Weakness in the hamstring muscles
  • Pain when stretching or contracting the muscle

What should I do if I have a pulled hamstring?

The first 24–48 hours are the most crucial in the treatment of a hamstring strain, as most of the bleeding and swelling within the injured muscle occur during this period. Limiting swelling helps reduce the extent of muscle damage. Immediate care should include:

  • Rest: Minimise use of the injured leg. In more severe cases, crutches may be required to avoid weight-bearing.
  • Ice: Apply ice to the affected area for 15–20 minutes every 1–2 hours. Crushed ice wrapped in a moist cloth is ideal, as it ensures effective cooling while protecting the skin.

While following these steps, it is important to contact your physiotherapist promptly to arrange an assessment and begin an appropriate treatment plan.

Physiotherapy plays a vital role in the recovery from a pulled hamstring. In the early stages, your physiotherapist will assess the exact tissue(s) affected and determine the severity of the injury. Based on this assessment, they can provide an accurate estimate of your healing time and develop a personalised treatment plan. Typical treatment plans often include a combination of techniques, such as manual therapy (manipulations) and electrotherapy, aimed at reducing pain and swelling while promoting optimal healing of the injured tissues. Other treatments may include:

  • Gentle stretching and progressive strengthening exercises to restore muscle length and power.
  • Soft tissue massage (avoiding the injured area in the early stages) to improve circulation and reduce muscle tightness.
  • Taping or strapping for support during early return to activity.
  • Gradual reintroduction to sport-specific drills under professional supervision.

What shouldn’t I do if I have a pulled hamstring?

In the first few days after pulling your hamstring, you should avoid any activities that increase blood flow to the injured area. These include hot showers, hamstring stretches, heat rubs, massage, alcohol consumption, and excessive use of the injured leg. Such activities can prolong bleeding within the muscle, worsen swelling, increase pain, and ultimately delay recovery.

Could there be any long-term effects?

With the correct treatment, most hamstring muscle strains heal within approximately eight weeks. However, some injuries may cause longer-term effects, particularly if the initial management is inadequate or the muscle damage is severe. In cases of significant tearing, additional structures in and around the hamstring may also be injured, further delaying recovery. Poorly managed hamstring injuries can result in a tight or weakened muscle group, increasing the risk of reinjury when returning to activity.
