Thoracic discectomy
A thoracic discectomy is a surgical procedure to remove a damaged or herniated intervertebral disc in the upper back (thoracic spine). Intervertebral discs normally act as shock absorbers between vertebrae, but over time, degenerative changes or wear and tear can cause a disc to bulge or prolapse. This can compress nearby nerve roots, leading to a band of pain around the chest, and in some cases, muscle weakness in the legs due to pressure on the spinal cord. Surgery may be recommended when severe pain does not improve with conservative treatments.
The procedure is performed under general anaesthesia, with an incision of approximately 3 cm made either through the back or the chest along the rib line, depending on the location of the disc prolapse. The surgeon removes the herniated disc fragment along with a small portion of bone and ligament to relieve pressure on the spinal cord and nerve roots. The primary goal of thoracic discectomy is to reduce pain, restore function, and prevent further neurological impairment.
Physiotherapy Before a Thoracic Discectomy
Pre-surgery physiotherapy focuses on strengthening and increasing the flexibility of your back muscles to support a smoother recovery. Your physiotherapist will also provide guidance on what to expect after the operation and how to prepare physically and mentally. At acephysiosports.com, our specialist physiotherapists can also offer non-surgical thoracic disc prolapse treatment to help manage symptoms and potentially reduce the need for surgery.
Symptoms Following a Thoracic Discectomy
After surgery, some pain and discomfort are normal, but this is managed with appropriate medication. Early movement is encouraged, and assistive devices may be used as needed. Your physiotherapist will guide you in safely getting in and out of bed, using the toilet, and walking, ensuring you mobilise correctly. Once you are safely moving and your pain is under control, discharge is usually possible within 3–5 days, depending on your individual recovery progress.
Physiotherapy Following a Thoracic Discectomy
At Acephysiosports.com physiotherapy after your thoracic discectomy is designed to help you regain independence in everyday activities and support a smooth recovery. Treatment is most effective when delivered by a specialist physiotherapist, and our team has extensive experience in post-discectomy care. Your rehabilitation programme will be personalised to your needs, with progress monitored through both short- and long-term goals. By focusing on the activities that matter most to you, physiotherapy will help maximise your functional potential and enhance your overall quality of life.
1–2 Weeks
In the first one to two weeks after surgery, physiotherapy focuses on reducing pain, improving comfort, and gently restoring movement. Treatment may include ice or ultrasound therapy, gentle stretching to improve spinal mobility, and hands-on techniques to relieve muscle tension and promote healing. Your physiotherapist will also teach safe transfer techniques, such as sit-to-stand movements, to boost confidence and independence in daily activities.
2–6 Weeks
Once initial healing has begun, your rehabilitation programme will progress to emphasise stretching, strengthening, muscle control, and aerobic fitness. Exercises will target the trunk, hips, and thigh muscles, with low-impact activities like walking or swimming encouraged to improve general fitness. Typical physiotherapy interventions at this stage may include:
6–12 Weeks
During this stage, therapy will focus on functional exercises to improve muscle strength, flexibility, and coordination in the upper back while supporting independence in everyday, work, and leisure activities. Emphasis will also be placed on correcting posture and varying positions throughout the day to prevent stiffness and fatigue.
12+ Weeks
Beyond twelve weeks, your physiotherapist will continue to tailor exercises around functional and work-related activities, building your confidence and ensuring safe, independent movement. Recovery time varies depending on individual symptoms and goals. Those with physically demanding jobs may require around six weeks before returning fully to work.
Throughout your rehabilitation at Acephysiosports.com, our physiotherapists aim to restore your independence and help you return to your previous activity levels as safely and efficiently as possible.
