What is a metacarpal fracture?
A metacarpal fracture refers to a break in one of the long bones within the hand, commonly known as the knuckle bones. This type of injury often results from direct trauma, such as punching a hard surface or falling onto an outstretched hand. Physiotherapy plays a vital role in the rehabilitation process after a metacarpal fracture. It helps restore movement, strength, and function to the hand, ensuring a safe and efficient return to daily activities or sports.
How can a metacarpal fracture occur?
A metacarpal fracture is commonly caused by punching a hard surface or from direct impact to the hand by a fast-moving object, such as a ball or blunt force.
What are the symptoms of a metacarpal fracture?
A metacarpal fracture usually causes sudden, sharp pain in the area between the knuckle and wrist. Swelling and bruising tend to appear quickly. If the broken bone shifts out of place, a noticeable deformity or bump may develop. Additional symptoms can include:
What should I do if I have fractured a metacarpal?
If you have or suspect a metacarpal fracture, you should seek immediate attention at your nearest accident and emergency department. To help control pain and reduce swelling before you arrive, apply ice to the injured hand. Use a bag of frozen peas or crushed ice wrapped in a moist towel, and apply it for up to 20 minutes. You may also apply compression using an elastic bandage. It should be snug but not so tight that it increases your pain. At the hospital, an X-ray will be taken to assess the bones in your hand. Based on the findings, the medical team will provide a diagnosis and determine the most suitable treatment. This may include immobilisation with a splint, or in some cases, surgery to realign and stabilise the bones.
Physiotherapy treatment following a metacarpal fracture.
Once the bone has healed, your physiotherapist will guide you through the process of returning to normal activities and, if necessary, safely resuming participation in sports. Treatment will focus on reducing any remaining pain and swelling, restoring joint range of motion, and performing stretching and strengthening exercises to rebuild function. Additional physiotherapy treatments may include:
What shouldn’t I do if I have fractured a metacarpal?
If you suspect a metacarpal fracture, avoid any activities that could cause the broken bone ends to shift before being assessed at the accident and emergency department. You should also avoid anything that increases blood flow to the area, as this can worsen bleeding and swelling and potentially delay recovery. This includes hot showers, heat rubs, massage, and alcohol consumption.
Could there be any long-term effects from a metacarpal fracture?
Most metacarpal fractures heal without complications within a few weeks. However, some cases may lead to longer-term issues, especially if nearby structures are also injured. These may include surrounding joints and cartilage, nerves, blood vessels, ligaments, or tendons. Damage to these structures can prolong recovery and may require more intensive treatment.
