Stress Fracture of the Talus

What is a stress fracture of the talus?

The talus is the foot bone that connects the leg to the lower ankle joint. A stress fracture of the talus is a small crack or incomplete fracture in this bone. Physiotherapy is an effective treatment to support healing, reduce pain, and restore normal ankle function.

How does a stress fracture of the talus happen?

Stress fractures of the talus occur when there is an imbalance between bone formation and bone breakdown. During activities such as running or other weight-bearing exercises, the talus is repeatedly loaded. Normally, the bone adapts by increasing turnover, but if the stress exceeds the bone’s ability to repair itself, tiny cracks develop, resulting in a stress fracture.

How does a stress fracture of the talus develop?

When stress is placed on the talus, small areas of bone can become damaged. The body resorbs (removes) the damaged bone and replaces it with new bone. If the new bone forms more slowly than the old bone is resorbed, weak points develop in areas of stress. Repeated loading of these weak areas can lead to a stress fracture.

Changes in training—such as increased frequency, duration, intensity, different surfaces, or new footwear—often contribute to the development of a stress fracture.

What are the symptoms of a stress fracture of the talus?

Stress fractures of the talus cause gradually increasing pain deep within the ankle over several weeks. Initially, pain may occur only after exercise. With continued activity, pain can worsen to the point where normal walking or even resting is uncomfortable, and the inside of the ankle may be sore at night. Other symptoms can include:

  • Weakness in the ankle or foot
  • Reduced balance
  • Changes in gait or walking patterns
  • Stiffness in the ankle

What should I do if I have a stress fracture of the talus?

If you suspect you have a stress fracture, contact a physiotherapist as soon as possible. Early assessment and treatment are crucial for proper healing.

What should I do if I have a stress fracture of the talus?

Do not continue exercising if you suspect a stress fracture. The area of the talus is weak, and continued stress can enlarge the crack or even cause a complete fracture. Rest and professional guidance are essential for recovery.

Could there be any long-term effects from a stress fracture of the talus?

If properly diagnosed and treated, a stress fracture of the talus usually does not cause long-term problems. Early intervention and addressing the underlying cause are key. Without proper treatment, there is a risk of the fracture worsening, a complete bone fracture, or additional stress fractures when returning to exercise.

Physiotherapy treatment following a stress fracture of the talus

Physiotherapy is essential for effective recovery. Your physiotherapist will first confirm the diagnosis, which may involve imaging such as an MRI scan. Based on this, a tailored treatment plan will be created. Initial management may include:

  • Rest and protection of the ankle, possibly using crutches
  • Ice therapy to reduce pain and inflammation

A programme will be designed to maintain cardiovascular fitness and muscle strength without delaying bone healing. Low-impact activities such as swimming, deep-water running, cycling, and cross-training may be included.

Your physiotherapist will also identify the underlying cause of the stress fracture and address it to prevent recurrence when you return to full activity. Additional treatments may include:

  • Electrotherapy to reduce pain and promote healing
  • Range of movement exercises to restore ankle flexibility
  • Hydrotherapy for low-impact strengthening
  • Balance exercises to improve stability and prevent future injuries
