ORIF Metatarsals Surgery

Open Reduction and Internal Fixation (ORIF) of the Metatarsals is a surgical procedure performed following fractures of the toes (metatarsals). Metatarsal fractures often result from traumatic injuries, and are commonly seen in athletes or individuals involved in high-impact sports.

ORIF Metatarsals Surgery Indications

The indication for ORIF of the metatarsals is significant displacement of one or more metatarsal fractures. Surgery is most commonly performed on the 5th metatarsal, as it is the most susceptible to injury.

Signs and symptoms of a metatarsal fracture are:

  • Pain and tenderness over the affected toe or metatarsal.
  • Swelling around the fracture site.
  • Bruising or discoloration on the top or bottom of the foot.
  • Difficulty or inability to bear weight on the affected foot.
  • Deformity or misalignment of the affected toe(s).
  • Pain worsened by walking or movement of the toes.

If you suspect a metatarsal fracture, you should have an X-ray performed. The results will help determine the extent of the fracture and guide the appropriate treatment plan.

ORIF Metatarsals Surgical Procedure

The ORIF procedure for metatarsal fractures is performed as follows:

  • Incision – A small incision is made over the affected metatarsal(s) to access the fracture site.
  • Fracture alignment (Open Reduction) – The fractured bone is carefully realigned to its normal anatomical position.
  • Temporary fixation – Wires or pins may be used to hold the bone in place temporarily.
  • Definitive fixation (Internal Fixation) – A metal plate and screws are inserted to maintain the alignment and stabilize the fracture.
  • Closure – The incision is closed with sutures or staples and dressed appropriately.
  • Post-operative immobilization – The foot may be placed in a cast or boot to protect the repair during early healing.

Post-Operative Care and Rehabilitation About one week after surgery, you will be placed in a cast. You will be provided with crutches and instructed to avoid putting weight on the operated foot for 6 weeks. After this period, you will transition to a CAM walker and begin protective weight-bearing for an additional 6 weeks. Physiotherapy rehabilitation will then commence to help you return to normal weight-bearing, regain full function, and safely resume sport or daily activities.

  • Pain management – Ice therapy (cryotherapy), gentle massage, and modalities to reduce post-operative discomfort.
  • Range of motion exercises – Gentle movements to restore flexibility in the toes, foot, and ankle.
  • Strengthening exercises – Targeting the muscles of the foot, ankle, and lower leg to regain functional strength.
  • Balance and proprioception training – Exercises to improve stability and prevent re-injury.
  • Gait training – Practicing correct walking patterns and weight distribution as you progress to full weight-bearing.
  • Scar tissue management – Techniques such as self-massage to enhance healing and mobility around the incision site.
