Hallux Valgus – Fusion Surgery (Arthrodesis)
Fusion surgery (arthrodesis) may be recommended to treat severe or recurrent bunions (hallux valgus).
Bunions, also known as hallux valgus, are enlargements of the tissues or bone at the base of the big toe, causing misalignment of the joint. In severe cases, this misalignment can lead to pain and limited range of motion, making surgery a potential treatment option.
Fusion surgery (arthrodesis) for a bunion may be considered if a previous Scarf osteotomy to realign the big toe has been unsuccessful.
Fusion surgery (arthrodesis) for a bunion involves making an incision on the top of the foot, extending from the big toe to the mid-foot. The big toe joint is then correctly aligned and secured using wires or a screw. This procedure effectively eliminates movement in the affected joint, permanently fixing it in place.
Although fusion surgery (arthrodesis) results in the loss of all movement at the big toe joint, the significant reduction in pain often makes the procedure worthwhile for many patients.
Physiotherapy is essential after bunion fusion surgery (arthrodesis) to help reduce pain, maintain strength, and re-establish a correct walking pattern.
Symptoms after surgery
Some pain is expected following fusion surgery (arthrodesis), but it can usually be managed effectively with pain relief medications. After the procedure, you will typically be required to wear a surgical boot for approximately six weeks to protect the joint and support healing.
Physiotherapy after surgery
0–6 Weeks Post-Surgery During the first couple of weeks, it is recommended to keep your foot elevated while resting to help reduce swelling. Ice therapy can also be applied to manage inflammation. While off your feet, it is important to keep your other joints moving to maintain strength and range of motion. Our physiotherapists at Ace Physio Sports will design a personalised exercise plan to maintain your stamina and preserve strength in the unaffected joints throughout your body.
6–12 Weeks Post-Surgery Once the surgical boot is removed, our physiotherapists at Ace Physio Sports will focus on retraining your walking pattern. Since the fused joint previously contributed to propulsion during walking, your gait may be altered. Once you are confident with your modified walking style, we will progress to more complex daily activities, such as climbing stairs, navigating uneven surfaces, and eventually returning to driving. Throughout this phase, our physiotherapists will prioritise your personal goals, working closely with you to achieve the outcomes that matter most.
12 Weeks and Beyond It typically takes 3–6 months before you can return to full function in recreational activities. At Ace Physio Sports, we will continue to support you, offering guidance and recommendations to help you safely return to the activities that are most important to you.
Summary
Fusion surgery may be used to treat severe or recurrent bunions, particularly if a previous procedure has been unsuccessful. The operation involves realigning the joint at the base of the big toe and securing it in place with wires or a screw. Once completed, the joint is permanently fixed, meaning there is no movement in the previously affected joint. Although fusion surgery is not ideal due to the complete loss of movement in the joint, it can be worthwhile for significant pain relief. At Ace Physio Sports, we support patients post-surgery with a tailored week-by-week plan to maintain strength during the time in your boot and to gradually work on your walking. Our physiotherapists continue to work with you until you are confident performing everyday activities and have achieved your personal recovery goals.
