Hallux Rigidus–Fusion Surgery
In hallux rigidus fusion surgery, the bone at the base of the big toe is joined to the first toe bone using screws, creating a rigid, stable joint.
Hallux rigidus is a type of degenerative arthritis affecting the joint at the base of the big toe. It is a progressive condition that leads to increasing stiffness over time. Pain and stiffness are often more noticeable in cold weather and tend to worsen with walking or activity.
Fusion surgery is typically recommended if you are experiencing severe joint pain and there is significant damage to the structures that allow normal joint function. This procedure effectively eliminates movement in the affected joint by permanently fixing it in place, providing stability and pain relief.
Although fusion surgery results in the loss of all movement at the joint, the significant reduction in pain often makes the procedure worthwhile for many patients.
Physiotherapy is essential after fusion surgery to help reduce swelling, manage pain, and re-establish a normal walking pattern, even though the joint’s range of motion has been permanently lost.
Symptoms after hallux rigidus fusion surgery
Some pain is expected following fusion surgery, but it can usually be managed effectively with pain relief medications. After the procedure, you will typically be required to wear a surgical boot for approximately six weeks to protect the joint and support healing.
Physiotherapy after hallux rigidus fusion surgery
0–6 Weeks Post-Surgery During the first couple of weeks, it is recommended to keep your foot elevated while resting to help reduce swelling. Ice therapy can also be used to manage inflammation. While off your feet, it is important to keep your other joints moving to maintain strength and range of motion. Our physiotherapists at Ace Physio Sports will design a personalised exercise plan to maintain your stamina and target the unaffected joints, ensuring overall fitness is preserved during your recovery.
6–12 Weeks Post-Surgery Once the surgical boot is removed, our physiotherapists at Ace Physio Sports will focus on re-establishing a normal walking pattern. Since the fused joint previously contributed to propulsion during walking, your gait may be altered. Once your walking technique is corrected and you feel confident, we will progress to more complex everyday activities, such as climbing stairs, navigating uneven surfaces, and eventually returning to driving. Our physiotherapists will prioritise your personal goals, working closely with you to achieve the outcomes most important for your lifestyle and mobility.
12 Weeks and Beyond It typically takes 3–6 months before you can return to full function in recreational activities. At Ace Physio Sports, we will continue to support you, offering guidance and recommendations to help you safely return to the activities that matter most to you.
Summary
Fusion surgery of the big toe is reserved for severe cases of arthritis, where the joint is so damaged that the supporting structures can no longer function effectively. While this procedure is highly effective at relieving pain, the permanent loss of joint movement means it is not always ideal. At Ace Physio Sports, we provide comprehensive post-surgical support, creating a tailored week-by-week plan to maintain strength during your time in the surgical boot and gradually progress exercises once the boot is removed. Our physiotherapists will continue to work with you until you are pain-free and confident in performing everyday activities.
