Hallux Rigidus–Cheilectomy Surgery
A cheilectomy is a surgical procedure performed to “clean up” the big toe joint by removing excess bone or bone spurs that may be restricting movement or causing pain.
Hallux rigidus is a type of degenerative arthritis that affects the joint at the base of the big toe. It is a progressive condition, causing increasing stiffness in the joint over time. Pain and stiffness are often more noticeable in cold weather and tend to worsen with walking or activity. In some cases, a visible bony lump may develop on the top of the big toe.
Non-surgical treatment can help delay—or in some cases even prevent—the need for surgery if hallux rigidus is identified early. Typical approaches include footwear modifications, pain management, and gentle exercises to maintain mobility and reduce discomfort.
If non-surgical treatments are unsuccessful and the other structures of the joint—such as ligaments, cartilage, and fat pads—remain in good condition, a cheilectomy may be recommended. If the joint’s other structures are significantly damaged, a fusion operation may be a more suitable option.
During a cheilectomy, a small incision is made over the big toe joint. The surgeon carefully removes the excess bone that is causing friction within the joint. Once the procedure is complete, the incision is closed using stitches.

Symptoms after surgery
After surgery, it is common to experience some pain around the scar site and residual stiffness in the joint. It is important to start gentle movement of the toe soon after the procedure to help restore full range of motion and prevent long-term stiffness.
Physiotherapy after surgery
0–1 Weeks Post-Surgery Immediately after surgery, elevation and ice are recommended to reduce swelling and discomfort. Towards the end of the first week, you will be provided with a special firm-soled shoe to help you walk safely. An exercise programme may also be introduced to maintain range of motion in the ankle and foot, as well as to strengthen the hips, knees, and ankles. Our physiotherapists will guide you through gentle passive exercises for your big toe to support early recovery.
1–2 Weeks Post-Surgery During this period, it is important to start moving and gradually bearing weight on your foot, as this helps restore mobility. Our physiotherapists at Ace Physio Sports will assess and correct your walking pattern, while also working on balance and stability. They will introduce an exercise programme to restore muscle balance in the foot, reducing stress and strain on the joint. Additionally, you will be taught how to manage your scar at home through self-massage techniques, which can help promote faster and healthier healing.
2 Weeks and Beyond At this stage, our physiotherapists at Ace Physio Sports will continue to focus on building your confidence on your feet and addressing any individual challenges or goals you may have, such as navigating stairs. Prior to discharge, your therapists will work with you at a pace suited to your recovery, ensuring a safe and gradual return to full function.
