Ankle Stabilisation

Ankle stabilisation is a surgical procedure used to reconstruct overstretched or torn ankle ligaments, a condition commonly referred to as ankle instability. When physiotherapy and strengthening exercises for the surrounding muscles have not provided adequate improvement, ankle stabilisation surgery is performed to repair the affected ligaments on the outer side of the ankle. This procedure aims to restore stability, reduce the risk of repeated sprains, and improve overall ankle function, allowing patients to return to normal activities safely.

Ankle Stabilisation Indications

Ankle stabilisation surgery is considered a last resort and is only recommended after conservative treatments—such as physiotherapy, bracing, and strengthening exercises—have failed. Surgery is most appropriate for individuals who have completed growth, ensuring long-lasting results. Common indications for ankle stabilisation include:

  • Recurrent ankle sprains: Multiple ankle sprains that have not responded to conservative management.
  • Chronic ankle instability: Persistent instability leading to functional limitations or difficulty with daily activities.
  • Ligament tears or significant ligament laxity: Confirmed by clinical examination or imaging.
  • Failure of physiotherapy or bracing: When non-surgical management does not restore adequate stability.
  • Risk of further injury or deformity: To prevent progressive damage to the ankle joint or surrounding structures.

Ankle Stabilisation Surgical Procedure

Ankle stabilisation surgery can be performed using different techniques depending on the severity of ligament damage. Ankle arthroscopy involves a small incision through which a fibre optic camera and surgical tools are inserted while you are under anaesthetic. This allows the surgeon to assess the joint, diagnose ligament damage, remove debris, or repair ligaments. Arthroscopy typically results in a quicker recovery and lower risk of complications compared to open surgery. Open ankle stabilisation surgery, performed under general anaesthetic, includes two main approaches. In direct ligament repair, the damaged ligaments on the outer ankle are repaired and reinforced to restore stability. In ligament reconstruction, a tendon graft—often taken from the patient—is used to replace severely damaged ligaments and stabilise the ankle. Both procedures aim to restore ankle stability, prevent recurrent sprains, and enable a safe return to daily and sporting activities.

After ankle stabilisation surgery, a below-knee cast, brace, or splint will be applied to protect the repaired ligaments. During this time, you may have a period of non-weight bearing, meaning you will need to avoid putting weight through the affected leg to allow proper healing.

The Benefits Post-Surgery

The success of ankle stabilisation surgery depends on the severity of the initial ligament instability and your commitment to post-operative rehabilitation. When performed effectively, the procedure can restore ankle stability and improve function. Benefits of ankle stabilisation surgery include:

  • Reduced risk of recurrent ankle sprains.
  • Improved joint stability, allowing safer movement.
  • Enhanced ability to participate in daily and sporting activities.
  • Decreased pain and discomfort associated with chronic instability.
  • Long-term protection of the ankle joint, helping to prevent further injury or degeneration.

Physiotherapy Rehabilitation Post Surgery

Physiotherapy following an ankle stabilisation procedure consists of a combination of techniques designed to help you return to normal daily activities and sports pain-free. Treatment is individualised to focus on achieving your personal goals and ensuring a safe, effective recovery. Post-surgery physiotherapy may include:

  • Range of motion exercises to restore ankle flexibility.
  • Strengthening exercises for the ankle, foot, calf, and surrounding muscles.
  • Balance and proprioception training to improve stability and prevent future injuries.
  • Gait retraining to normalise walking patterns.
  • Functional and sport-specific exercises to return you safely to your chosen activities.
