Whiplash

What is whiplash of the lower back?

Whiplash of the lower back occurs due to a sudden, sharp movement of the trunk—forward, backward, or sideways. This injury affects the muscles, tendons, and ligaments in the lower back, similar to whiplash of the neck. Physiotherapy plays a crucial role in the treatment and recovery of lower back whiplash.

How does whiplash of the lower back happen?

Whiplash of the lower back is most commonly caused by road traffic accidents. The sudden, forceful movement pushes the lower back beyond its normal range, stretching muscles, tendons, and ligaments. Small joints in the lower back may also become injured during this motion. Other causes include falls from height or lifting heavy objects.

What are the symptoms of whiplash of the lower back?

The most common symptoms of lower back whiplash are pain, swelling, and stiffness. The area may be very tender to touch, and surrounding muscle spasms can make movement difficult and painful.

  • Pain radiating into the buttocks or legs
  • Numbness or tingling in the lower back, buttocks, or legs
  • Reduced range of movement in the lumbar spine
  • Weakness in muscles of the lower back or legs
  • Difficulty standing or walking for prolonged periods

What should I do if I have whiplash of the lower back?

Most symptoms of lower back whiplash usually last up to a week. However, it is important to visit your GP or the nearest hospital, depending on the nature of the accident, as there may be more serious injuries that require treatment. Applying ice to the area can help reduce swelling; this can be done by wrapping frozen peas in a wet towel and placing it on the skin for up to 10 minutes at a time. Ice can also help alleviate pain. Your GP may prescribe painkillers or anti-inflammatory medications to help you move the area as comfortably as possible.

Physiotherapy treatment for whiplash of the lower back.

Some symptoms of lower back whiplash may persist for several months before fully subsiding. In all cases, it is crucial to begin a physiotherapy programme as soon as possible to optimise recovery. Physiotherapy will include cryotherapy (ice) to reduce swelling and massage to minimise muscle spasm. Range of movement and strengthening exercises will also be incorporated to improve mobility and reduce stiffness in the lower back. Other treatments include:

  • Postural advice and ergonomic adjustments to reduce strain on the lower back
  • Heat therapy to relax muscles and improve blood flow
  • Electrotherapy techniques such as TENS or ultrasound to relieve pain
  • Core stability exercises to support the lumbar spine
  • Hydrotherapy or gentle cardiovascular exercises to maintain overall mobility
  • Education on activity pacing to avoid overloading the injured area

What shouldn’t I do if I have whiplash of the lower back?

Ignoring symptoms of lower back whiplash can prolong recovery and may lead to long-term issues such as reduced mobility and muscle weakness. It is important to avoid heavy lifting and repetitive movements until your symptoms have fully resolved. This approach promotes effective healing and helps minimise the risk of re-injury.

Could there be any long-term effects from whiplash of the lower back?

The longer symptoms of lower back whiplash persist, the harder it becomes to return to full fitness. Physiotherapy is especially helpful in managing these more chronic, long-term symptoms and supporting a complete recovery.
