What Is Shoulder Instability?

Shoulder instability occurs when the ligaments and capsule that support the shoulder joint become stretched or loose, reducing the joint’s stability. How Does Shoulder Instability Happen?

Shoulder instability occurs when the ligaments and capsule that support the shoulder joint become stretched or damaged, allowing excessive movement. It most commonly follows a shoulder dislocation, where the top of the arm bone is forced out of its socket. This injury overstretches and damages the surrounding capsule and ligaments, reducing their ability to stabilise the joint.

Shoulder instability can also develop over time from repetitive stress, such as frequent throwing motions, which gradually loosen the capsule and ligaments. Additionally, some individuals have naturally lax ligaments, a condition often referred to as being “double-jointed,” which can lead to instability in multiple joints, including the shoulder.

Symptoms of Shoulder Instability

Shoulder instability can cause the bones within the shoulder joint to slip or partially dislocate, a movement known as “subluxation.” This often produces a clunking or popping sensation, sometimes accompanied by deep shoulder pain. As a result, you may unconsciously avoid moving your arm into positions that trigger the clunking.

Some people experience a “dead arm” sensation, where the arm feels numb or weak for a few seconds following a subluxation. In severe cases, where the ligaments and capsule are very loose, the shoulder may dislocate repeatedly. Other common symptoms include pain, weakness, reduced range of motion, and feelings of instability during everyday activities or sports.

Rehabilitation typically requires several months of dedicated physiotherapy, which is often very successful. However, in some cases, instability may persist, leading to repeated dislocations that can damage surrounding structures such as nerves, bones, and joint cartilage. Damage to the cartilage may contribute to shoulder arthritis later in life. Surgery may be recommended to tighten the capsule and ligaments and restore stability if ongoing laxity persists.
