What is referred lower leg pain?

Referred pain occurs when discomfort originates in one area of the body but is felt in another. In many cases, pain from the lumbar spine (lower back) can be referred down to the lower leg, a condition known as referred lower leg pain. This type of pain often results from irritation or compression of nerves in the lower back. Physiotherapy is an effective treatment, aimed at addressing the underlying cause and relieving symptoms through targeted exercises and manual therapy.

How does referred lower leg pain happen?

The spine is made up of individual bones called vertebrae, which serve as the structural building blocks of the spinal column. At each vertebral level, spinal nerves exit the spine and travel to various parts of the body to supply muscles and skin. The lumbar vertebrae (located in the lower back) give rise to nerves that supply the lower limbs. If these nerves become compressed or irritated, pain may be referred to the lower legs. Common causes include arthritis, trauma, spinal stenosis, and disc prolapse. Less common but more serious causes include infection and malignancy.

What are the symptoms of referred lower leg pain?

There are many symptoms associated with referred lower leg pain. These may include:

  • A dull, aching or burning pain in the lower leg
  • Pain that radiates from the lower back or buttocks down into the leg
  • Numbness or tingling in the leg or foot
  • Muscle weakness in the lower limb
  • Increased pain with certain movements, such as bending, lifting, or prolonged sitting
  • No obvious injury to the leg itself

What should I do if I have referred lower leg pain?

If you recognise symptoms of referred lower leg pain, you should consult your GP as soon as possible. They may prescribe pain relief and arrange further investigations to determine the underlying cause. These may include MRI scans, X-rays, or CT scans. Physiotherapy can begin as soon as symptoms are identified, helping to manage pain and improve mobility while addressing the root cause of the issue.
