What is costochondritis?

Costochondritis (or Tietze’s syndrome) is the inflammation of the cartilage connecting a rib to the breastbone (sternum). This condition often causes localized chest pain that may worsen with movement, deep breathing, or pressure on the affected area. Physiotherapy can play a key role in managing and resolving symptoms of costochondritis. Treatment may involve gentle mobilization techniques, posture correction, breathing exercises, activity modification, and modalities such as ultrasound or TENS to reduce pain and inflammation.

How does costochondritis happen?

Costochondritis is commonly caused by a direct blow to the joint between the rib and breastbone (sternum). This can happen during contact sports, such as tackling in rugby or football, or from an impact to the chest during ball sports. It can also develop over time due to excessive ‘wear-and-tear’ of the joint. This is more likely in individuals who regularly perform activities involving significant trunk and rib cage rotation—such as rowing, heavy lifting, or repeated twisting motions—placing repetitive stress on the costosternal joints.

How does costochondritis feel?

Costochondritis typically causes central chest pain that is aggravated by physical activity, deep breathing, or certain movements of the upper body. The pain may be sharp or aching in nature and is often located on one side of the sternum, though it can sometimes affect both sides. Additionally, the area around the affected costosternal joint is usually tender to touch, and pressing on the chest wall can reproduce or worsen the pain.

What are the symptoms of costochondritis?

There are many different symptoms associated with costochondritis. Potential symptoms may include:

  • Sharp or aching pain in the front of the chest
  • Pain that worsens with deep breathing, coughing, or sneezing
  • Pain that increases with physical activity or upper body movement
  • Tenderness when pressing on the affected ribs near the sternum
  • A feeling of tightness or pressure in the chest
  • Pain that may radiate to the back or abdomen
  • Discomfort when lying down or changing positions
  • Pain typically affecting one side of the chest, but can occasionally be bilateral

What should I do if I have costochondritis?

If you have or suspect you have costochondritis, you should arrange a physiotherapy assessment. Early diagnosis and treatment can help manage the symptoms effectively and prevent the condition from becoming persistent. A physiotherapist can confirm the diagnosis, identify contributing factors such as poor posture or overuse, and create a tailored treatment plan to relieve pain and improve function.

Physiotherapy is very important in the treatment of costochondritis. Initially, your physiotherapist can perform an assessment and diagnose your problem. Following this assessment, your physiotherapist will devise a treatment plan. Treatment may involve activity modification, the use of anti-inflammatory medications, and electrotherapy treatment. Other treatments include:

  • Manual therapy to reduce stiffness in the thoracic spine and rib joints
  • Postural correction exercises to relieve strain on the chest wall
  • Breathing techniques to reduce tension in the chest and improve rib cage movement
  • Stretching exercises targeting tight muscles around the chest and shoulders
  • Strengthening exercises to support the trunk and reduce stress on the rib joints
  • Education on ergonomics and lifestyle modifications to prevent recurrence

What shouldn’t I do if I have costochondritis?

If you have or suspect you have costochondritis, you should not ignore the problem or continue with activity as normal. Doing so may lead to your condition worsening and significantly prolong your recovery. It's important to avoid any activities that aggravate your pain

Could there be any long-term effects from costochondritis?

Costochondritis does not typically produce any long-term effects, especially when it is properly diagnosed and managed. However, full recovery can take several weeks or, in some cases, even months.
